I have never had a successful orgasm through oral sex. What's wrong with me?

Hi-thanks for writing in with your question. Not to worry! Most people don't orgasm under all sexual conditions. Some people can only orgasm in intercourse, but not during oral sex. Some people can only orgasm in oral sex, but not during intercourse. Some people can orgasm in neither of these situations and only orgasm through manual stimulation. Some people can only orgasm in solitary sex, some only during partnered sex. Everyone is different in this regard.
From what you write, I can't tell for sure if you are able to ejaculate under other conditions, but it sounds as though you are. If so, that's ok. That could just be your normal pattern. If you are not able to ejaculate through oral sex, intercourse, manual stimulation (or under any conditions), then you need to consult a physician.
In males, orgasm and ejaculation are two seperate physiological processes that occur almost simultaneously, most of the time, for most people. However, they are really two different things. Some men even train their bodies to have multiple orgasms without ejaculating just so they can feel even more pleasure before they release their ejaculation with their last orgasm.
I also noticed that you are on testosterone therapy. Is this because your testosterone levels are low? If so, this could be having an impact on your ability to ejaculate. Having the right amount of testosterone in your body (whether a person is male or female) is an important part of the ability to respond sexually Low testosterone levels make it harder to climax.
