Question:
My husband has been masturbating for years, and I don't have a problem with self-pleasure, as long as there is no danger and it is not a replacement for sexual relations together. However, in recent months, his technique has been to insert things directly into his penis. Though alarmed, I thought "who am I to say what feels good for him" and I would have left it at that, if I had not witnessed on more than one ocassion, that he stimulates himself until blood (and sometimes it appears to be a goodly amount) is present, then ejaculates. I am concerned about him. I perceive this "normal" (masturbation itself)activity to have crossed the boundaries of safe and sane sexual practices. In addition, as he does not want to wear a condom during intercourse with me, and has inserted... who knows what into his body, is there or are there ongoing risks to MY health by his actions?

Bleeding through the urethra is a sign of urethral injury. Possible complications might include introduction of infection into the urethra or a urethral stricture. I thus do not find your husband’s technique to be safe and advise him to visit a urologist to evaluate for possible complications. I also advise psychiatric consultation and marriage counseling. Best wishes, Hussein Ghanem, MD
