What you are most likely feeling is an area of sensitivity collectively referred to as 'The G Spot' (Gräfenberg Spot) This term came about after the publication of a book in 1982 which referred to a 1950 article in the International Journal of Sexology in which Dr. Ernest Gräfenberg wrote about erotic sensitivity along the anterior vaginal wall. Here is a good article with detailed information and the original article by Gräfenberg and a diagram that shows the location of that spot When palpating this spot you are also feeling the Skenes Glands and glands that enclose the urethra. For women who are able to ejaculate at the point or just a split second before full body orgasm - it can feel like they either have a strong urge to urinate or are actively passing a small amount of urine, because of the excitability of the urethra, the engorgement of the tissue and the stimulation of the glands. These glands can release suddenly to almost 'squirt' out the fluid. As for reliably finding the 'spot' every time, and getting the same result can be unpredictable. Her mood, time of month, bodily position, health and other factors will determine whether or not she wants and is able to have what is referred to as a G Spot orgasm, and whether or not she ejaculates. For some women ejaculation is only an occasional occurrence, but a very pleasurable one.
