Question:
My girlfriend has recently had problems with vaginal tearing (like a mini-episiotomy) during sex. This has only been a problem since I moved out of state and we have only been able to see each other once or twice a month. When we were living in the same town, we had sex much more frequently. Is there anything we can do to prevent this tearing during sex? We have tried extended foreplay and lubricants, but they haven't seemed to help.

Females are, generally, capabable of accomodating the penis during sex. It is csutomary to feel somewhat tight at the beginning then, with regular intercourse, things get better (she gets 'used' to it as some say. If she is tight to the degree that causes tears, I think she may need to see a gynecologist, may be she needs a small incision that helps her, especially that you said you tried extensive forplay. Now, there is one thing though. There are commercially available 'dilators'. These may be used regularly when you are not togther, to keep her on the wider side until you meet again. If she can get a set, she may try it for 4-6 weeks before she entertains the idea of the operation.
