as erectile dysfunction (ED) and ejaculation problems (premature ejaculation,
or PE, and delayed ejaculation, or DE). This chapter will focus on the psychological
components of those types of dysfunction with a discussion of how relationships
and partners are often affected. Even though the chapter focuses on men, many
factors, such as the importance of sexual scripts and the impact of a person’s
life history, apply to women as well. Heterosexual and homosexual couples are
affected by the same interpersonal dynamics, and whenever heterosexual couples
are mentioned in this chapter, those scenarios apply just as well to homosexual
relationships. Case examples are presented throughout the chapter; these case
examples are fictional.
ED and other male sexual dysfunctions can be caused or influenced by a number
of biopsychosocial components. It was not too long ago, and with little proof,
when professionals felt that as many as 90 percent of cases of male erectile
dysfunction had their origin in psychological issues and conflicts. The pendulum
has shifted today to placing greater emphasis on exploring biological causes
for sexual dysfunctions. Steady progress is being made in this area as new
diagnostic techniques and medical treatments are becoming more successful.
Despite the progress in medical diagnosis and pharmacological intervention, practitioners
continue to be aware of the important role that psychological and relationship
issues play in the development and continuance of sexual dysfunctions. This is
true even if the dysfunction is caused by medical conditions. There is considerable
emotional impact on the man and his relationships even if the sexual dysfunction
has a direct biological cause. Regardless of the roots of the problem, male sexual
functioning is closely aligned with self-identity. The personal impact of sexual
dysfunction is real. Any discussion of sexual dysfunction must keep in mind the
